Once you have submitted an online booking request, within 72 working hours, one of our operators will phone you to fix your appointment. Our operators will make three attempts to contact you by phone. If they do not manage to reach you, they will send you a text message.
Only one appointment per online request.
To book appointments for more than one person, even if the contact details are the same, please fill out a separate request for each patient.
To make more than one appointment for a patient, please fill out a request form for each appointment.
For forms submitted with more than one request, only the first request will be handled.
Please note: This form cannot be used to request blood tests and surgical procedures through the National Health Service.
